Illness or Misadventure
This form is for students in Stages 5 and 6 to make an application for special consideration of examinations, tests and submitted assessment tasks due to illness or misadventure.
Should I submit an illness/misadventure application?
An illness/misadventure application is for students who, for a valid reason:
are late in submitting a task,
fail to complete the task on the day assigned day,
are absent on the day of the task or examination, or
are requesting an extension to a task.
Students must complete and submit an illness-misadventure application within five school days of their return to school.
Illness: If you were sick or injured or had a diagnosed medical condition affecting your performance or causing you to miss an examination, assessment task or submit a task.
Misadventure: An incident or circumstances affecting your performance in examinations or assessments tasks or prevented you from submitting a task by the due date.
For an application to be considered, it should be accompanied by a medical certificate or report, a written statement from a parent, guardian or health professional and/or any other relevant documentation.
The application will be based on evidence supplied by the student and in other submitted documentation, and will be made in consultation with the Head of Academic Innovation and at least one of the following:
Subject Teacher
Wellbeing Facilitator/Leader
Academic Leader
Administration Coordinator
Head of Secondary
Head of School
Once the application has been reviewed, the student will be contacted regarding a decision and the nature of any subsequent arrangements, such as extensions and/or alternative dates. Usually, students will sit for in-class tasks or examinations as soon as possible after they return to school (in most cases on the day they return) and, depending on the nature of the assessment, an alternate task may be set.
What reasons are valid for an illness/misadventure application?
Illness supported by a doctor’s certificate
Family disruption due to serious illness, death or misadventure to a family member
Accident in the period leading up to the due date of the assessment task
Participation in a school sanctioned activity, such as representing the school in sport, debating etc., provided prior arrangements are made for the completion or submission of the task.
What reasons are not acceptable for an illness-misadventure application?
Forgetting that the task was due
Inability to complete the task due to the pressure of other school work (prior application has to be made in these circumstances)
Driving lessons or tests, job interviews and non-urgent medical and dental appointments
Work commitments outside school (assessment tasks take priority)
Computer or printer breakdown
